So many factors went into this story but the main theme is how change can be frightening. This book appeals to teens and adults as well as children who only generally get the literal meaning. When you really get into it, you'll see that Nelle, the fearful Caterpillar needs to be in control because she can't except that she's going to change. When she loses that control she stops eating. The subtle suggestion here is that this story reflects an eating disorder but kids won't get that, they'll focus more on the metamorphosis and the fear associated with that.
